Foglio membranaceo del sec. XVI (mm. 610 x 410), vergato recto/verso con notazione musicale quadrata su tetragramma in inchiostro rosso, al recto grande iniziale miniata che segna l'accesso all'inizio del canto gregoriano "Sacérdos in aetérnum Chrístus Dóminus", iniziale "S" entro quadrato dipinto in rosa e bianco, con corpo della lettera in oro con motivi decorativi in blu, all'interno su uno sfondo paesaggistico si erge la figura di S. Domenico in abito talare con l'ostensorio nella mano destra ed il giglio nella sinistra, raffinata grande bordura laterale fogliacea sui toni del blu, rosa e verde con rialzi in oro, al verso iniziale dipinta in rosa ed oro, qualche leggera macchia e ondulatura della pergamena, entro cornice in legno, probabilmente del sec. XVIII, rivestita in velluto ocra con piedistallo, sciupata e qualche mancanza.
